SHP Provider Portal
Registration Form

SHP Provider Connection Portal is a secure environment for physicians and practice staff to access Sentara Health Plans transactions. Please complete the registration form below for each potential SHP Provider Portal user. Username and password information is CONFIDENTIAL and per HIPAA guidelines cannot be shared.

After the registration form has been submitted, please allow up to 10 business days to process your request. The user will receive a secure email with a username and temporary password to log into SHP Provider Portal.

Please check your SPAM and save svc-pp-class3mgmt@sentara.com as a trusted email to ensure the follow-up emails are received.

Users will have instant access to member eligibility, benefits, and claim information within 2 business days after receiving login credentials. Access to remits will become available within 4 business days.

I acknowledge that the user below is an authorized representative of this practice/facility. I agree to notify Sentara Health Plans promptly of additions or deletions of users. I understand that with the implementation of on-line health plan information, our practice will begin receiving documents, manuals, directories and bulletins from Sentara on-line, instead of by mail and/or fax.

Paper remits will not be provided when the provider begins to receive payments and remits electronically by means of EFT (Electronic Fund Transfer), ERA (Electronic Remittance Advice) or eRemits.

New User: Select if you do not have an existing login for our secure SHP Provider Connection Portal.

Account Reactivation: Select if (1) you have not signed into “SHP Provider Connection Portal” within 90 days or (2) you have forgotten your username for "Provider Connection".

Account Change: Select by users with an existing active SHP Provider Connection Portal account who's unable to access Tax ID due to (1) Change practice or (2) Add an additional Practice(s); Tax ID or vendor number.

The "New User" option should be selected if you do not have an existing login for our secure provider portal - Provider Connection.

The "Account Reactivation" option should be selected if (1) you have not signed into “SHP Provider Connection Portal” within 90 days or (2) you have forgotten your username for "SHP Provider Connection Portal". The "Account Change" option should be selected by users with an existing active Provider Connection account who are unable to access Tax ID due to (1) a change in practice or (2) adding an additional practice(s), Tax ID, or vendor number.
